ToeJam and Earl Back in the Groove has been the fourth main game in the series. The gameplay of the game is very simple and is based on exploring and searching up floating islands. However, the game is played in a different style as compared to the other video games in the series. This 2D world game is played from the 3 and 4 perspectives. The islands will generate randomly throughout the game that also floats in the space. The islands are connected to each other through elevators. Some islands also contain the spacecraft wreckage of which player must collect 10 in order to win the game.

The game mechanics has been based on rouge-like and dungeon crawl game. The player will drop to an island below. The islands has been populated by different items such as ice cream trucks, nerds, hamsters, bogeymen, man-eating mailboxes and chickens which are armed with mortars and fire tomatoes. All such things will aid the players in the game. Overall the game is non-violent as the character will only attack enemies with tomatoes, randomly generated power-ups.

Besides the game also featured the Jam Out rhythm based mini-game. There are over 9 playable characters in the series and four-player co-op gameplay both online and locally. Power-ups are in wrapped presents in the game and differ in term of their appearance. Each present has a unique effect while some items will increase the different parameters of the player such as speed while they will also distract enemies. The NPC can also identify presents in lieu of a small fee. The player can hold an item at random.
